Promises and hazards of gene therapy: giants or windmills?

Abstract

After 25 years of ups and downs, gene therapy has reached adulthood as a scientific discipline and we no longer say that it is an attractive future possibility but rather a reality that is healing and improving the quality of life of an increasing number of patients. This winding road, however, has been accompanied by a parallel bioethical debate that is closely related to the peculiarities and the very essence of this discipline. Many of the initial fears about what was then known have already faded, but others have emerged such as the realization of the dangers and risks of the use of oncogenic integrative vectors, the high costs of the first products that have reached the market or the cosmetic use of gene therapy tools. More recently, in light of the discovery of new and efficient tools for gene or genome editing, the old debate about the possibilities of germ-line gene therapy has reemerged, a subject that has been taboo and indeed explicitly prohibited in most countries that have legislated on this subject.
